CBCL/l½-5 TEMPLATE FOR HAND-SCORING

ITEM #604 Optional Item: Facilitates the scoring of Item 601- English form only

Reusable templates to make transferring item scores from CBCL/l½-5 forms to hand-scoring profiles easier. (Same for both genders.) Helpful optional aid; reusable cardboard cut-out that helps with the transfer of item scores from forms completed by respondents to hand-scoring profiles. Used with items 602 and 603.  USING TEMPLATES ALONE WILL NOT HAND-SCORE FORMS; USE WITH HAND-SCORING PROFILES. 

The numbered problem items on the template do not align perfectly with the current form version due to spacing differences.  Users may slide the template vertically to align the problem item numbers.
